Dr. Nolan will take new patients who join the Signature MD Concierge Program.

We are accepting new Medicare or Medicare Replacement Insurance Patients. 

We do not accept any Medicaid/Passport Insurance plans, nor do we accept Workers Comp or Motor Vehicle Accident patients.

PAYMENT

All copays are due at the time of service. If you fail to pay at that time you will be charged an additional billing fee of $10. We accept VISA, MC and Discover.

If you have a balance, you may phone in your payment if paying by credit card at 451-9296. If you now have a deductible instead of a copay you will be required to put down a deposit of $25.00 towards that deductible balance. This does not apply to Medicare patients

For patients with no Insurance we will discount the office visit if you pay in full at the time of service. This discount does not apply to Lab tests.

 

If your Insurance has changed or you received a new card, please show your card to the front desk when you sign in. You must tell the receptionist immediately upon signing in, before leaving the window area (She may not ask). It is your responsibility to make sure that we have the correct information before your seen. If we fail to get this updated information the wrong insurance could be billed and thus resulting in your receiving a bill.

If your address, phone numbers or work number has changed please tell us when you sign in.

 

FORM FEES

A minimum charge of $50.00 will be charged for forms to be filled out. It may take up to 48-72 hours before they are ready.

NO SHOW/CANCELLATION POLICY

Please allow us 24 hours notice if you need to cancel an appointment. This will allow us to fill your spot with another patient that needs an appointment.

This is greatly appreciated.

The patient will be charged a fee of $50 for missed appointments. As a courtesy, we call the day before to remind you of your appointment, but it is your responsibility to remember your appointment

MEDICATION/REFILLS

If you need a refill on your medications, please have your pharmacy send us the request-it makes the refill process much smoother and quicker that way. We use e-prescribe which is a nationwide electronic prescription service. It is better for us if your pharmacy sends the request.

If you need a new prescription please call the office with the name of your medication and the pharmacy name and phone number where you want it sent.

Requests may take up to 48 hours to process, but most requests are processed the day we receive them. At times there may be a lot of requests, so please allow any extra time you will need so you do not run out of these medications. Our daily cutoff time for refills is 2:00 pm on Mon, 3:00 pm on Tues-Thurs and noon on FRIDAYS. The office is closed on Saturday.

Please let us know if your are needing 90 day prescriptions or 30 day prescriptions.

Bring your medications with you to your visit or an accurate list of all your medications, including medications from other Physicians.

Unfortunately we cannot replace lost or stolen narcotic medication, nor will these be refilled on the weekends.

If you need a control medication you will need to be seen every 3 to 6 months to continue getting these medications. (This is now a law that we have to abide by). Please allow at least 48 hours for the Doctors to send in your medication electronically.

FOLLOW UP APPOINTMENTS

We often have you come in for fasting labs prior to your visit. Please take your medications with water prior to arriving for that fasting visit.

If the Provider wants you to make another appointment, please do so at the time you check out to make sure an appointment will be available for you.  Physicals and Pap appointments often times must be booked far in advance.

Occasionally we have Student PA's that will see you in addition to the Provider you are scheduled with. This would only be done with your consent.

Labs are done on a first come first serve basis. We ask that you arrive between 8am and 11:30am for fasting labs and between 9am and 1130 am for non fasting labs, all afternoon labs are from 1:00 pm to 4:00 pm. We ask that you be patient with your wait as it does take time for the LabCorp person to get all the information entered and blood drawn.

REFERRALS

If your are in need of a REFERRAL you must contact Kristin Willibaum at Ext 5 at least 1 week before your scheduled appointment with the specialist. She will need the Name of the specialist, as well as, the date and time for your visit.

**We do our best to see patients in a timely manner but in a doctor's office emergencies can sometimes cause delays and result in us not seeing you at your scheduled time. Please be patient with us as we go through this process.

If you are unable to wait we will be glad to reschedule your appointment.
